Rural communities have difficulty recruiting and retaining primary care providers. With the number of aging providers looking at retirement and the lack of medical school graduates interested in primary care, the need will be further increased. The national shortage of primary care providers has many communities searching for help. Meeting the current needs is difficult and the need for services is expected to increase as the rural population ages and the enactment of the patient protection and affordable care act. Advanced practice registered nurses, whom studies show can provide care that is safe and as effective as a physician, are a powerful solution to the problem of the shortage in primary care providers and rising health care costs. Analysis of categories and themes determined that three of the concepts of rural nursing theory were applicable. It was anticipated that the knowledge gained would contribute to understanding nurse practitioner choices to practice in rural areas, to determine the developing rural nurse practitioner educational programs and existing federal agencies involved in recruiting and retainment nurse practitioners, in rural areas. A qualitative study using a focused ethnographic approach was used to explore the cultural construction of rural nurse practitioner roles. A conceptual model analysis from the theory of cultural marginality was developed. The marginality framework was developed to provide a basis for data collection and analysis. Data set used interviews conducted using audio/video tapes of electronic media interviews through web conferencing, except two interviews were conducted by telephone. Data analysis was through transcription of interviews; at the end of the analysis three concepts from the rural nursing theory were identified. Lack of anonymity was subdivided into respect and annoyance. Outsider versus insider status was defined, not as being an insider in the community, but as an outsider in the medical community. Permission to conduct the study was granted by the University of Texas at El Paso’s IRB. Names of participants did not appear anywhere in the report or research. This study identified the three concepts of the rural nursing theory. In the study, the nurse practitioners did not find the lack of anonymity to be an annoyance, but viewed it as a part of being an effective medical care provider. The study indicated that nurse practitioners are well accepted by the community members. However, nurse practitioners are not always accepted by the existing medical community. Self-reliance was indicated by the nurse practitioner’s ability to function alone and make independent decisions related to their clinical practice. Due to differences between urban and rural care needs, it is important that nurse practitioner students be given an appreciation for the uniqueness and benefits of rural practice including a) limited access to healthcare, b) limited resources, and c) distance to specialists. This can be accomplished through changes in didactic and clinical components of nurse practitioner educational programs. Nurse practitioners who accept positions in rural areas most often have a background in rural areas, have participated in a rural training program, or have a desire to serve in a rural community. Completion of a rural program is associated with accepting a position in a rural practice. In this study, 72% of the nurse practitioners completed at least one rural clinical practicum. This supports the importance of rural training as an effective strategy in recruitment of nurse practitioners for rural areas (Boynton Sharp, 2010). In another study published by Purdue University (Richards, 2011) stated that as practitioner students are subjected to underserved populations, they are able to better understand the complicated clinical situation, which also confronts nurse practitioners to provide valuable and professional, high quality safe and cost efficient patient care. The practitioner students rotated through the clinics; this was found to improve the staff role in clinical knowledge, strengthen the evidence of clinically relevant research, and enhance the classroom instructions and assignments. The rotations made class instruction and assignments more meaningful when the students observed the instruction being used in a clinical setting. The experience integrated previous subject matter and allowed practitioner students to develop skills in a supportive and strengthened environment. Allowing students to perform alongside nurse practitioners, help to develop mentoring and educational relationships, regard for others capability, and improves comprehension of the nurse practitioner role. The staff roles shifted from the expert to a mutually supportive learning association (Richards, 2011). The advanced practice nurse can serve to fill the gap in primary health care. Nurse practitioners can provide quality, cost effective care to these communities, offsetting the limited access and rising health care costs. American Political System and Philosophy Essay

The United States of America would have been completely different had the Founding Fathers been influenced by a different philosophical stream. Had democracy not been the cornerstone of the nation, Americans would not be enjoying the rights they have today. Democracy, in its literal meaning, suggests that the power of the state rests on its constituents and that, therefore, the fate of the country solely depends on its people. Without a democratic system, there will be no elections and no representations in the political arena. Since elections provide the means for the people to constantly shape the course of the nation’s institutions through elected officials, the lack thereof deprives Americans of their right to steer their country towards the direction which they see best fits their needs. For the most part, the Founding Founders perfectly embodied the noble principle of democracy in three simple words—â€Å"We the People†. If the political philosophy of Thomas Hobbes became the foundation of America, Americans will be forced to surrender their rights to a single authority. In effect, Americans will be left powerless as the â€Å"Leviathan† would have now held and wielded the aggregate of all their rights. The limitation of the political philosophy of Hobbes is clear—it does not recognize a large number of the rights of the people, more so their inalienable rights. Without recognizing the rights of the people, society becomes either lifeless or mechanical as individuals are forced to follow whatever the sitting authority dictates. In the society of Hobbes, Americans have no right to tell the ruler what to do. In some cases, modern Americans are unable to convince their local officials to do as they are told by their constituents. Rather, some local officials play deaf and mute to the calls of their constituents to make certain actions that will favor the larger body politic instead of a few chosen elites. If the political philosophy of John Stuart Mill became the foundation of America, the political system would have been chosen depending on the greatest good for the greatest number. In short, the political system will essentially be a Utilitarian system. In such a system, certain harms towards some Americans are permissible so long as the majority of the population is able to gain the best benefit. It then becomes clear that human rights violations are permissible if such violations would realize the best interest of the people. For example, the murder of thieves vigilante groups commit becomes acceptable because it reduces the possible sources of threats to life and property to most Americans. In the long run, the rule of law becomes parallel to the rule of the majority. Minorities who stand in opposition to the principles and ideals of the majority become powerless and are reduced to â€Å"toothless† citizens of the nation unable to change the social situation. Oftentimes, modern American culture reflects the Utilitarian perspective. One compelling example is the time when African-Americans were racially discriminated. Their discrimination was left unattended to by the federal government at that time primarily because most of the American population such as the â€Å"conservatives† benefitted from the oppression—benefit as in the context of their perception. I think the political philosophy of John Locke best reflects the formation of America as a nation. Locke primarily espouses the idea that men in the State of Nature would come together and transfer the power to punish transgressors to the government. Although the will of the majority requires all people to follow that will, the government plays the role of judge in times when offenses towards the people occur. The society formed from convention of people comes with certain laws that guide the actions of all people and serve as an adjudicating force with the aid of judges. Today, much of Locke’s political philosophy can be observed in America. The country has its own set of laws that guide the conducts of its citizens. Although the will of the majority is observed especially during elections, it does not necessarily deprive the minority of their rights and privileges in the society. As a matter of fact, American laws, in principle, do not discriminate between those who belong to the majority and those who are from the minority. All people are equal before the law.

How accurate is the film Gladiator Essay

Gladiator is a wonderful story and won Best Picture in 2000. Even though the movie is so well done it does not mean that the story is historically accurate. The movie includes many historically accurate facts ranging from some of the characters to the society of Rome. However, Gladiator is for pure entertainment, and just like the real gladiator battles, it is for the enjoyment of the viewers. The film added to the cinematic categories such romance and intrigue but at the same time took away from many historical aspects. Even though the movie is on the whole historically inaccurate there are a few historically accurate details. In Gladiator, the hugeness of the Coliseum is very well and accurately portrayed. The Coliseum is larger then Shea Stadium, and that greatness and grandeur is shown correctly in the film. Also, the movie correctly portrayed the armory of the times. The breastplates were correct and so too were the facemasks and other such battle suits.( http://www.online-shrine.com) The movie shows these true details in order for the viewer to think that the other more amazing parts of the plot are true. Also, by adding the truths it makes sure that the movie will not seem neither phony nor cartoonish. The reason Gladiator can not be considered historically accurate is because of the glaring errors in the characters and the emotions and actions of the characters. Firstly, Crowe’s General Maximus Decimus Meridus is a total fake. He is not a real person nor is he very similar to any one else in history. Maximus is instead a hodgepodge of many other people. The one true person who is totally shown incorrectly is Commodus. Commodus is shown as a grownup that never outgrows his childhood. However, he in fact was not like that at all. In the film Commodus is shown killed is father because his father believed he wasn’t fit to rule. In truth Commodus did not kill his. Historians generally agree that, Marcus Aurelius died of the plague in Vienna on March 17, 180 CE (www.nmia.com). It is also untrue that Marcus found his son unfit to rule. He had Commodus named Caesar when he was 5 years old, and named Commodus as his successor when he was seventeen. As a Roman father he undoubtedly loved and spoiled his son terribly. Gladiator is not a historically accurate film and is filled with hogwash. The film can not be used to study that time period and can only be used as a great Hollywood film but nothing more. Regardless of the original message that Robert Frost had intended to convey, his poem, â€Å"The Road Not Taken†, has left its readers with many different interpretations. It is one’s past, present, and the attitude with which he looks upon his future that determines light that he will see the poem in. In any case, this poem clearly demonstrates Frost’s belief that it is the road a person chooses that makes him the man he is.
